Our Vision

Evexia Health (Luton GP Federation) strives to be an established and respected entity embedded within, and supporting the development of, a thriving place-based health and care system designed around the Primary Care Networks (PCNs) and individual general practice registered lists.

 

 

Our Mission

 The Evexia team work in collaboration with GP practices, PCNs (and other providers) to offer caring, safe, effective high quality services by sharing resources and expertise, and a leadership team allowing headspace to innovate.

Working together, we will deliver proactive primary care ‘at scale’ -  increasing General Practice resilience and improving the health and wellbeing of our local population.

 

Our Aims

  • Support a whole system approach to delivering integrated care
  • Focus on improving population health and reducing health inequalities
  • Improve patient experience offering good access to high quality primary care services, reducing unwarranted variation
  • Reduce workload pressures for GP practices, and help to make General Practice a more attractive option for staff
  • Support the development of primary care leadership at place and Integrated Care Board (ICB) level
