What is a federation?

Federations are organisations to support “primary care at scale”; with the aim of remodeling care/  community interface; and to integrate with other health and social care functions. The concept of a Federation as  integrators of care in the community aligns completely with the direction of travel outlined within the 5-Year NHS Forward View.

Federations were first discussed within the RCGPs in 2007 in “the RCGP road Map” and development has been under way since, some of the federations are well established and others are just starting up but all have broadly similar aims. The focus of the RCGP roadmap was to encourage GP practices to work together and share expertise and be able to deliver services at a wider scale than just their own patient groups. Since then, Federations have been forming across the country in response to the roadmap.

Evexia Health covers 23 GP practices and a population of 205,000+ patients across Luton

The specific aims of the federation are to:

• Support a whole system approach to delivering integrated care
• Focus on improving population health and reducing health inequalities
• Improve patient experience offering good access to high quality primary care services, reducing unwarranted variation
• Reduce workload pressures for GP practices, and help to make General Practice a more attractive option for staff
• Support the development of primary care leadership at place and ICS level

Why was Evexia Health established?

Nationally there is an increasing challenge for general practice, both in terms of clinical workload and issues with staff shortages. The increasing patient need and a rapidly changing healthcare economy is leading to an environment that is strained, unable to evolve and potentially unsustainable.

Evexia Health Ltd was created by the practices to provide them with the capacity to evolve and meet modern healthcare needs. It aims to be a mature provider utilising the existing workforce to reduce variation , deliver better quality care and improve patient services.

General practice has huge capacity to adapt/evolve and deliver quality services when given the chance. Luton has a passionate core of practices already delivering quality services and eager to do more under the right conditions.

Our Role

Our role in this is to develop ideas with the local practices, health care providers and managers to facilitate local transformation.

The Evexia team currently offer a range of services and appointments with GP’s and other Healthcare Professionals. The service is available to patients registered with a local GP Surgery from Luton. An appointment with these services is just like having an appointment at your own GP Surgery.

Your medical record will be available to the Healthcare Professional who sees you, to ensure that they have the information they need to give you the best possible care. The GP or Healthcare professional you see will update your record with full details of the consultation that has taken place to ensure that your medical record is up to date.

Innovation

We are working on a number of projects at the moment to provide more provision on a regular basis to our patient population.

Evexia Health is a company limited by shares

Practices have contributed £0.01/per patient to establish the federation. Member practices hold shares depending on the size of the population but each has equal representation and voting rights.
